KUDOS FOR THE MORONGO BASIN COMMUNITY HEALTH CENTER

The Morongo Basin Community Health Center recently received national recognition for four programs it offers. The Health Resources and Services Administration, a division of the Department of Health and Human Services, identified the Take a Hike Program, Community Resource Directory, LIFT transportation services, and Coalition Formation as four programs that will be presented to the HRSA for potential duplication at clinics throughout the United States. The Community Health Center was also awarded continued funding to improve the quality of care at the center.

  • Take A Hike Program: As part of a National Parks initiative to expand community use of local national parks, the healthcare District partnered with the Joshua Tree National Park to create the ā€œTake A Hikeā€ program. Medical providers throughout the Morongo Basin can write a prescription for patients to ā€œTake A Hikeā€ in the Joshua Tree National Park. The prescription is valid for 90-days and provides free entry to the park to encourage physical exercise and improved health.
  • Community Resource Directory: A directory of community partners is managed by CHC staff for resources such as transportation, food access, educational opportunities, health and wellness activities. The concise, four-page document is distributed to patients who use CHC services.
  • LIFT Transportation Services: The free LIFT Transportation program is operated by the healthcare District to provide improved access to medical and preventive care for Morongo Basin residents.
  • Coalition Formation: This practice focuses on creating partnerships within the local community to avoid duplicating resources and identifying collaboration opportunities. The practice includes conducting ā€œlistening toursā€ to interview potential partners and identify strengths and weaknesses in services, and creating work groups to target specific community health needs.
